Family History Assessment Module (FHAM)
The Family History Assessment Module (FHAM) was created by individuals working at Washington University as part of the COGA project (collaborative
studies on genetics of alcoholism).
It is designed to assess major psychiatric disorders (drug
dependence, alcoholism, mania, depression, antisocial personality, and
schizophrenia) in relatives of the person being interviewed.
Eleven questions are asked, and for every question that is answered
positively, the interviewer
records the name of the relative and their relationsip to the
participant. Each positive answer is also followed up with an
individual assessment module (IAM) for each family member that was
listed in the FHAM.
