Anxiety Experiences Questionnaire (AXEQ)

The
Anxiety Experiences Questionnaire (AXEQ) is comprised of three measures: the Penn State Worry Questionnaire, the Phobia Questionnaire developed by Marks and Matthews, and the DSM-III R criteria for Panic Disorder. The Worry Questionnaire yields a Total Worry score. The Phobia Questionnaire yields a Total Phobia score, and 3 scale scores (Agoraphobia, Blood-Injury Phobia, and Social Phobia). The DSM-III R criteria yield 7 scores (presence of panic attacks, panic attack symptoms, unexpectedness of panic, incidence of panic in past month, incidence of panic in lifetime, panic disorder criteria, agoraphobia criteria).
